COVID-19 PRECAUTIONS

Due to Covid-19, the surgery is posting out forms to patients, to minimise attendance to the practice. Please phone the practice at 01 4730893 to arrange for forms to be posted. 

Patient Forms

Patient forms are completed by GPs outside of surgery hours. Forms should be dropped into reception for completion by the GP. These forms will be ready for collection after 48 hours. 

Sick Certifcates

Patients who require sick certificates on a weekly or monthly basis as directed by the GP should request the certificate from reception by phone the day before it is due for collection.
